SECTION 341(a) MEETING OF CREDITORS - TAPE DUPLICATION REQUEST

Written transcripts are not available for Section 341(a) Meetings.  Instead, you may order a duplicate audio tape recording of the meeting. The cost for each tape is $20.00 which does not include sales tax or postage. Special handling increases the cost, as noted in the payment section below.  To place an order, complete this form and mail to the below address. The duplicating company then sends you the duplicate of the tape along with the bill.
